Registration fee includes uniform (jersey, hat, socks), end-of-season award, and insurance. Multi-child discount: $25 off each additional sibling.

Provide player information, parent/guardian contact details, and select a division.

After submitting the form, you will be directed to our secure payment page to complete your registration.

Confirmation

You will receive a confirmation email with details about the upcoming season, including team assignments and practice schedules.

  • ✓Parent/guardian contact information (name, email, phone)
  • ✓Emergency contact name and phone number
  • ✓Any medical notes or allergies we should know about
  • ✓Jersey size preference (Youth S/M/L/XL or Adult S/M/L)
